AREA L AHEC: Practice Support Consultant


This is a Contract position in Fort Collins, CO posted May 9, 2022.

Position Summary: Provide support to ambulatory care practices, providers, and staff in rural and underserved areas to enable them to thrive in the rapidly changing healthcare marketplace.Assist providers with implementing systems to reduce costs and improve patient clinical outcomes.

Provide on-site training and technical assistance on the tools and systems used by providers in value-based payment activities, provide assistance with navigating Medicaid Managed Care, Clinical Workflow Redesign, Community Health Worker Implementation, EHR and HIE Optimization, COVID-19 resources, and Practice Operational Assessments.

Coordinate continuing professional development (CPD) activities related to practice support and quality improvement (QI).

Duties and Responsibilities:Serve as a Practice Support (PS) Consultant, provide visits to physician practices that are involved in quality improvement projects, provide technical support, Continuing Professional Development (CPD), training, and education as warranted to meet practices’ needsPerform required monthly practice assessments and ensure tracking of data is entered and up to date in designated databaseWork with Area L AHEC Practice Support Director and the NC AHEC PS leadership team at the NC AHEC Program Office to assist in the identification and development of resources, educational programs, and reports to support PS/QI processes and initiativesAttend training meetings, conference calls, and web-based programs pertaining to QI and PS as neededParticipate in relevant blogs, list serves, and other resource platforms to rapidly disseminate information within the NC AHEC PS statewide programCommunicate with and educate practices regarding tools and resources to support practices’ work to achieve QI goalsDevelop project plans, timelines, and achieve required milestones as they pertain to current NC AHEC PS contract projectsServe as the liaison between participating practices, Area L AHEC, NC AHEC Program Office, and other stakeholders as neededWork closely with Area L AHEC Director of Practice Support and Director of Medical Education to provide direct support to those practices that need assistance with implementing QI projects and to implement practice-based Continuing Medical Education (CME) activities related to QIMonitor the marketplace and communicate with providers and practices about current trends, needs, and emerging modalities in the delivery of healthcare educationIdentify educational needs at the practice and community level to plan, coordinate and provide CPD for healthcare professionals and constituentsEducation and Experience:Post baccalaureate credential in health or administrative field and 2 years’ experience in healthcare quality improvement or healthcare management in an ambulatory practice setting; OR a baccalaureate degree in a related field and 5 years’ experience in healthcare quality improvement or healthcare management in an ambulatory practice settingQualifying degrees must be received from appropriately accredited institutionsAbility to work a flexible schedule with travel in the regionAbility to communicate effectively in writing and/or verbally with individuals at all levels as well as possess team building and conflict resolution skills; Comfortable with publicspeaking & presenting Additional Required Skills and Abilities:Knowledge of federal, state, and independent payor quality programs that have an impact on ambulatory practices, e.G., MACRA, MIPS, Medicaid Managed Care Performance Improvement Projects (PIPs)Knowledge of innovative and population health-based models of care and associated recognition programs, e.G., PCMH, PCSPExperience with evidence-based quality and process improvement; measurement and data monitoring; organizational and change management; health care reform payment models and programs; and adult learning principles and educational best practicesDemonstrated knowledge of continuous quality improvement techniquesFamiliarity with process mapping and work-flow analysis toolsAbility to manage multiple projects and activities on a routine basis, with minimal supervisionEasily adapt to changes in both individual project(s) and health care environmentExperience with at least two EHR vendor products, use of mobile devices, printers, copiers, fax machines, calculators, and computers.

Proficiency in, or high level of comfort in learning, Word, Excel, Access, PowerPoint, E-mail, and Web based applicationsExperience in coaching or teachingKnowledge of primary care practice environments as well as theories of practice & organizational changeSkills in QI methods, Project Management, Data driven changeExperience in an ambulatory care practice Valid NC Driver’s License and good driving recordAbility to manage confident and sensitive information appropriately Should be well organized, self-directed, able to work independently, with the ability to interact, in a professional manner, with staff, faculty, and the publicMust be able to manage multiple responsibilities and meet deadlines
