Per Diem, Various.Pay range is between $29.56/hr to $39.60/hr, depending on years of relevant and like experience.

The Case Manager is responsible for providing children and their family’s access into evaluation/treatment and coordination of care within the Pediatric Rehab department.

Case Managers who work full time are expected to maintain a productivity rate of up to 10 intakes per week.

This process may include phone or email contact with physicians, obtaining prescriptions for services from providers, reviewing insurance benefits, making recommendations for evaluation and treatment, reviewing service recommendations with families, providers and third-party payers as well as supporting families to make informed choices regarding use of insurance benefits and financial resources.

The Case Manager must strive to promote teamwork within the Peds Rehab departmentally and work professionally with physicians and individuals in the community.JOB QUALIFICATIONSEducation or Formal TrainingPreferred experience in a health and/or human service-related profession such as: early childhood, child development, medical administration staff, medical scheduling and/or medical billing.Special Qualifications (Licensure, Registration, etc.)NoneKnowledge, Skill and AbilityRequires the ability to understand therapy care needs for a diverse population of pediatric rehabilitation patients; knowledge of normal child development from birth – 18 years is helpful; knowledge of pediatric rehabilitation therapies and their role in treatment; knowledge of the practice of case management; ability to identify andprioritize evaluation and treatment needs of the patient in collaboration with the child’s family; knowledge of Medicaid, Colorado no-fault insurance, and other third party payer guidelines.

Must have good judgment, problem-solving, oral and written communication skills and time management skills.

Must have clear articulation; aided hearing acuity must be adequate to comprehend speech spoken at normal loudness level; manual dexterity to operate computers and office equipment and ability to effectively assist patients and their families in making healthcare choices.ExperienceMinimum of 1-year experience in case management or healthcare setting preferred.WORKING ENVIRONMENT/PHYSICAL ACTIVITIESPosition requires frequent use of telephone, sometimes for long periods of time with frequent interruptions, usually sitting position.

Sitting at a desk with frequently typing or writing.

May stand when using copy or fax machine.Position interacts with a variety of patients and families.Physically able to work 8 hours a day.Moderate to high stress level caused by frequent exposure to high needs patients and situations, busy work environment, rapidly changing situations, interruptions and schedule changes.Occasional stressful encounters with staff, patients, families and physicians.INTERRELATIONSHIPSCommunicates effectively with physicians, patients, families and staff regarding patient status.Ability to problem-solve and deal with diverse populations.Demonstrates sensitivity to public relation implications when dealing with patients, visitors, health team members and the public.Functions as a team member and represents self professionally.Responsible for knowing how to access adequate and appropriate clinic services for patients and families with Low English Proficiency.ESSENTIAL DUTIES AND RESPONSIBILITIESDemonstrates appropriate Encore!

Complete scheduled phone intakes.Manage an assigned caseload of patients starting with the intake process; Contact parent or personal representative by phone to review reason for referral and summarize history as needed to make evaluation/treatment recommendations.Review insurance benefits with family during phone intake.Generate an intake summary in the patient’s electronic medical record.Uploading documentation into electronic medical record.Schedule needed evaluations.Review evaluation recommendations.Obtain insurance authorization for treatment when needed.Ensure prescription from referring provider is on file at start of services.Communicate with schedulers to ensure timely scheduling of treatment following evaluations.About Boulder Community Health and Boulder, ColoradoUncompromising medical excellence delivered with respect and compassion.
