This is a Full-time position in Fort Collins, CO posted September 15, 2021.

DescriptionSummary of Position: Provides services to address the social and/or emotional factors that impact the client’s ability to achieve the goals of the Plan of Care.

Services are provided under the direction of a physician and in conjunction with other health care team members in accordance with the established Policies and Procedures of the facility.

Coordinate pediatric patient services with the interdisciplinary team, patient/family and referring agency; and assumes the responsibility for coordination of care. Essential Duties and Responsibilities These include the following: (Other duties may be assigned) Performs education, and ongoing evaluation of clients who are receiving services under a medically approved Plan of Care.Completes assessments in a timely manner and in accordance with Facility Policy and Procedure.Develops a written plan to provide information and direction to other health team members.Documents all findings, plans, interventions, and client progress in the clinic record.Communicates plans and changes to the physician and/or nursing supervisor and other Facility staff through the Care Plan, written progress notes, and participation in care conference.Prepares social histories to augment existing services, or a guide in determining or changing level of service, taking into consideration.Family relationshipMeaning of illness to the client and/or his/her familyCultural attitudes in relations to illnessThe financial impact of client’s illness upon the familyDemonstrates team work and effective communications to accomplish client, team and Facility goals.Participates in case conferences and other team or Facility meetings.Educates team members and clients/families about community resources and how to assess them appropriately.Serve as a preceptor and mentor for clinical peers, you will report directly to the branch director or regional clinical director and serve as a peer leader/mentor to new hire clinicians and other peer clinicians.

You will interact with patients as well as other clinicians day to day included but not limited to performing in home assessment/care visits with the new hire clinicians.Orient new clinicians which includes clinical home policies and procedures, documentation, technology, compliance and regulatory requirements.Facilitate the learning experience to allow the new clinician to perform competently in their required responsibilities.

Does this by mentoring, coaching, and role modeling correct behaviors.Provides learning experiences to promote the acquisition of new knowledge, skills and abilities required of the job.Provides timely feedback to the new clinicians regarding questions of the orientation experience.Provides clinical skills practice opportunities for new clinicians and evaluates and documents correct performance during clinical skills evaluation process.Performs patient home visits in accordance to physician’s ordersExpectation to participate in on call rotation Performs other related duties and responsibilities as deemed necessary. Education and Experience Specifications:o Registered Nurse with the Colorado State Board of Nursingo Current Texas Driver’s License, good driving record, and reliable transportation, valid and current auto liability insurance.o Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursingo CPR Certificationo Computer skills including, but not limited to, MS Office, MS Excel and Scheduling program.o Interpersonal, organizational and excellent written and verbal communication skills.o Ability to carry out and understand written and oral directions, to read and write.o Maturity and ability to deal effectively with the demands of the job. Must be able to cope with the mental and emotional stress of the position.

Must function independently and have flexibility, personal integrity and the ability to work effectively with clients, personnel, and support agencies.

Must be able to relate to and work with the ill, disabled, emotionally upset, and, at times, hostile people within the agency.Home: Works in patients home in various conditions; possible exposure to blood and bodily fluids and infectious diseases; ability to work flexible schedule; ability to travel locally; some exposure to unpleasant weather, PRN emergency call.
