UCHealth: Care Manager Rn-Medical Center Of The Rockies


This is a Full-time position in Loveland, CO posted October 8, 2020.

Summary:Works collaboratively with physicians, interdisciplinary teams, individualclients and families to promote positive client outcomes.

Performs a carecontinuum process that assesses, plans, implements, coordinates, monitors andevaluates the options and services required to support the individual’s healthneeds utilizing skilled communication, education, and resources to promotequality, cost-effective outcomes.

Provides focused support to various areassuch as utilization management, emergency department, acute, ambulatory andspecialty care.

Work Schedule: PRN Days Responsibilities:Demonstrates critical thinking skills when utilizing the nursing process, basedon research, evidence-based outcomes and Standards of Practice to meet client’shealth care needs.Establishes collaborative partnerships with clients to assist them in examiningpatterns of health care needs, decisions, lifestyle choices, and utilization ofresources that affect their health.Advocates, educates and coaches clients, the family and/or caregiver abouttreatment options, community resources, psycho-social concerns in order to setgoals and help the client develop self-care skills and independence appropriateto their age and developmental level.Facilitates communication and coordination between members of the health caredelivery team, involving the client in the decision-making process in order tominimize fragmentation in services.

Within scope of job,requires critical thinking skills, decisive judgement and the ability to workwith minimal supervision.

Must be able to work in a fast-pacedenvironment and take appropriate action.Requirements:Graduate of an accredited Registered/Professional Nursing program if less than 3 years’ experience.

State licensure as a Registered Nurse (RN).

Basic Life Support (BLS) Healthcare Provider.2 years’ nursing experience.

Preferred: Acute care and 1 year Case Management experience.Preferred: Bachelor’s degree in Nursing.Preferred: Certified Case Manager (CCM).

BLS through the American Heart Association or the American Red Cross CPR for the Professional Rescuer with card in-hand before start date.

BLS or CPR card must be good through sixty days of hire.

All life support certification cards must be issued by either the American Heart Association or the American Red Cross CPR for the Professional Rescuer.
