Oversees and coordinates all aspects of placement for individuals being provided with new services, Shared Living/Host Homes, and other residential placements.

Assists in the coordination of temporary respite services.

Provides interim program management.Essential Job Functions:Engage with a variety of sources to build a referral base including but not limited to schools, churches, community members and case management agencies.Responsible for the recruitment and interviewing of SharedLiving/Host Home providers by using a variety of recruitment methods, screens applicants, conducts background screenings, and completes application process.Completes home inspections and HUD survey (as applicable) in Shared Living/Host Home settings.Gathers and relays additional information from all applicable eligibility determination entities or organizations, other service agencies, family/guardians, etc.

to the Program Manager.Meets with individuals (and participants of their choosing) to determine needs, preferences, and personal goals and assists with the coordination of the person centered planning process.Assists Shared Living/Host Home providers and Program Managers in the coordination of temporary Respite services.Provides residential program management duties for all new Shared Living/Host Home service recipients for the first thirty to ninety days or for the frequency needed.

Completes necessary assessments and completes inventory of personal possessions.Provides training and support to the Shared Living/Host Home provider.

Puts together the initial master, home, and/or medical files.Maintains a database for referrals and placements.Other duties as assigned.Supervisory Responsibilities:During the placement period and 30 days after placement, this position is responsible for establishing and implementing program goals, arranging or providing orientation and training, providing direct oversight of providers, maintaining a quality assurance program, developing and maintaining individual budgets, providing an environment that is conducive to the welfare of the service recipients and is in accordance with federal, state, local and organizational regulations.Minimum Job Qualifications:Education and/or Experience: Bachelor’s Degree in human service or related field and two years experience working with individuals with developmental disabilities.Certificates, Licenses or Registrations:Must have a valid driver’s license and a clear driving record; must have cleared and approved criminal record check; must have legal status in the U.S.; must hold First Aid/CPRcertification.Other Competencies (skills, abilities, behaviors):Must possess a commitment to the belief that all people have the right to dignity, respect, opportunity and full community inclusion.Must have a philosophy that is consistent with the Mission, Vision, and Core Values of the organization.Must be able to read, write, and comprehend English effectively.Must have ability and commitment to maintain confidentiality.Must be a positive team player and maintain a positive attitude; skills in planning, organizing, problem solving, and conflict resolution.BLC123
