Patient Home Care RN Coordinator

This is a Contract position in Boulder, CO posted September 11, 2021.

Patient Centered RN
– Home Coordinator Health Division Grade: NE14 Summary Responsible for the application of extensive knowledge in clinical performance improvement activities.

Supports and ensures the overall success of a patient-centered medical home by collaboratively working with patients, clinicians, practice teams, and the healthcare system.

Duties and Responsibilities: Provides direct, professional nursing care to patients utilizing the nursing process of assessment, planning, implementation, and evaluation.

Delivers patient-centered care to patients and families by providing care coordination and care management services.

Responsible for driving engagement strategies for improving clinician and staff patient care practices based on clinical guidelines, best practices, and internal and externals benchmarks.

Educates individuals and families on recommended services and exams that are due across their lifespan.

Participates in and leads quality improvement projects and initiatives within the medical department.

Supports self-management of disease with individuals and families Incorporates advanced care planning with patient, family, and healthcare team Promotes clear communication amongst care teams and treating clinicians by ensuring awareness of comprehensive care.

Complies with all of the instituted health division policies and procedures applicable to job position.

Ensures strict confidentiality of health records.

Provides care in a culturally sensitive manner to the population served Requirements (Education, Experience, Knowledge, Skills, Abilities): Bachelors in Nursing required; will consider Associates Degree in Nursing with certification in care management and/or as a certified diabetic educator.

Current Registered Nurse license for the state of Wisconsin 5 years’ experience in a patient care setting of patients across the lifespan is required.

Certification in BLS required Maintains flexibility and willingness to rotate work days to cover patient care during clinic hours Monday through Saturday Ability to provide care in a practical manner according to training, experience and approved competencies.

Experience with delegation of approved tasks.

Knowledge and experience providing care coordination and care management services Knowledge of health education theory and practice Knowledge of chronic conditions and preventative care for patients across the lifespan Knowledge of applicable federal, state, county and local laws, regulation and requirements Researches and shares evidenced based nursing practice and assists with implementation as appropriate Build care relationships among patient, family and team; support the primary care-giving role of the family with appropriate Ability to work in collaboration with Nursing Director, department supervisors, and Health Educator to ensure patient educational materials are consistent, up to date, health literate and available for the diagnostic categories prevalent with the population served at the FCPHWC Experience working with electronic medical record systems Valid Wisconsin driver’s license required; must maintain a valid Wisconsin driver’s license as a condition of employment Must show proof of immunizations required for FCP Health Division employees and successfully pass a background check and pre-employment drug screen
