Position: Case Manager, CO-SLAWLocation: Fort Collins, COPosition Status: Full timePosition Summary:*Support persons enrolled in MAT tocontinue engagement with both medical and behavioral health treatment plans.*Provide appropriate clinicalassessment to be integrated into the initiation of MAT.*Monitor members according toguidelines set by NCHA Policy and Procedure under the CO-SLAW guidelines.*Participate in telephone contacts,office/hospital visits, and other means of connecting with members to reachidentified goals.*Enrollment of patients (members)using appropriate assessment tools & completion of GPRA Qx at enrollmentand then at regular 3-month follow up intervals (up to 12 months) within the 7treatment sites where DATA/CARA waivered providers are already in existence.*Perform health needs assessments andidentify risk factors to develop health care plan*Assist members in schedulingappointments, follow-up care, referrals, medication refills, etc.*Attend member appointments withproviders and community resources as needed.*Participate in health promotion andhealth education activities for members as identified in health care plan.*Enter all documentation, contacts, andassessments into required databases.*Optimize member and familyindependence through education, community resources, and support.*Review appropriate cost-effectivecare and decrease duplication of services for members.*Understand, communicate, andfacilitate on behalf of patients with regard to complaints, grievances, andappeal processes.Wage Range: Between $17.59/HR and $24.62/HR.Requirements:Bachelor’s degree in a Human Services field from an accredited college oruniversity; experience in the behavioral health field.Preferred: Bilingual/Bicultural;Experience with Trauma-Informed Care; CAC II, III or LAC a plus.SummitStone is dedicated to promoting diversity, multiculturalism, and inclusion.
